Medical Social Worker, Hospice, Full time

📋 Description

• Make home visits to clients in designated geographic territories.
• Assist our team in understanding the social and emotional factors related to our clients’ health
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify appropriate community resource referrals to address client and family needs.
• Advocate for clients and families to navigate the community system.
• Participate in developing the total plan of care with the multidisciplinary BAYADA team.

🎯 Requirements

• Must be licensed by the NJ Board of Social Work Examiners at the LMSW or LISW level.
• MSW with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by school transcript or degree.
• Home care experience is a plus.
• Ability to read, write, and communicate in English.
• Ability to travel to cases as assigned.
