Medical Social Worker, (MSW) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to patients' health issues.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resources to address patient and family needs.
• Advocate for clients and families to navigate community systems.
• Collaborate with the multidisciplinary BAYADA team to develop care plans.

🎯 Requirements

• Minimum of a Master’s in Social Work with at least 1 year of health care experience.
• Council on Social Work Education accreditation demonstrated by transcript/degree.
• Home care experience a plus; able to read, write, and communicate in English.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills; prior EMR experience preferred.
• Ability to travel to assigned cases.
