🎯 Requirements

• Masters in Social Work; 2+ years in community-based setting
• CSWE accreditation demonstrated by transcripts/degree
• Home health experience required
• Strong English communication; independent work skills
• Computer skills; EMR preferred
• Willingness to travel to cases
