New Grad Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements on mobility, strength, balance, and safety.
• Apply wound care interventions and evidence-based dressings as needed.
• Develop and implement individualized care plans with therapeutic exercises and gait/balance
• Assess and revise PT care plans; collaborate with nursing in multidisciplinary plans.
• Educate clients and families on rehabilitative care to promote independence.

🎯 Requirements

• PA Physical Therapist license (current).
• Minimum 1 year PT experience.
• Graduation from APTA-approved program or CAAHEA/AMA accredited.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
