New Grad Physical Therapist (PT), Full Time, Home Health

📋 Description

• Visit clients in their homes within designated geographic territories.
• Provide PT treatment to help recovery after injury or surgery.
• Assess mobility, strength, balance, and safety.
• Develop and revise individualized care plans; include manual therapy as needed.
• Document observations and interventions using a touchpad tablet.
• Collaborate with nursing and the multidisciplinary care team.

🎯 Requirements

• Active and current Physical Therapist license.
• New graduates welcome; strong interest; one year of PT experience preferred.
• Home health experience welcomed, but not required; training provided.
• Ability to travel locally to cases as assigned.
