New Graduate- Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, balance, gait, and safety assessments.
• Implement wound care with evidence-based dressings and topical agents.
• Develop individualized PT care plans with manual therapy and gait training.
• Continuously assess and revise the PT care plan with nursing.
• Educate clients and families in rehabilitative care and safety.

🎯 Requirements

• A current MD Physical Therapist - PT license.
• Minimum of one year of PT experience.
• Graduate of an APTA-approved or AMA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid EMR experience or computer skills.
