New Participant Advocate

📋 Description

• Builds trusted relationships with new participants and caregivers to understand individual goals
• Coordinates the participant onboarding journey by serving as the primary point of contact during
• Partners with the Interdisciplinary Team (IDT), caregivers, Referral Coordinator, and
• Advocates for participant preferences, cultural needs, and person-centered care by proactively
• Provides education and guidance to participants and caregivers regarding Habitat Health services
• Coordinates participant appointments, transportation, follow-up services, and other onboarding

🎯 Requirements

• Bachelor's degree in Social Work, Human Services, Healthcare Administration, Public Health
• Minimum of one (1) year of experience working with older adults, frail elderly populations
• Demonstrated ability to build trusted relationships while delivering compassionate
• Demonstrated ability to coordinate services, navigate complex participant needs, and support
• Strong communication and collaboration skills, with the ability to effectively engage participants
• Strong organizational skills with the ability to manage multiple priorities while maintaining
