Nurse Care Manager

📋 Description

• Support care delivery within the Evergreen Aligned (EGA) model with APPs.
• Ensure effective execution of individualized CKD/HTN/diabetes/CHF care plans.
• Provide education, remote monitoring, and medication management.
• Coordinate care through labs, referrals, and follow-up communications.
• Conduct pre-visit planning and reinforce APP-directed plans.
• Promote patient engagement and outcome improvement in a virtual setting.

🎯 Requirements

• Active, unrestricted RN license.
• 2+ years of clinical experience in CKD, diabetes, or hypertension.
• Experience with EHR (electronic health records).
• Excellent communication; bilingual as applicable.
• Ability to work remotely with minimal supervision.
• Experience in chronic disease management (CKD/diabetes/hypertension).
