Nurse Practitioner

📋 Description

• Provide ongoing primary and preventive care to older adults
• Manage and grow a dedicated patient panel in assigned communities
• Deliver relationship-driven, patient-centered care in community settings
• Collaborate with interdisciplinary clinical and operational teams
• Support preventative care, chronic disease management, and care coordination
• Utilize telehealth and in-person care delivery to improve access

🎯 Requirements

• Active, unrestricted Nurse Practitioner
• Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology)
• Active DEA license and CS registration as required
• 3+ years of clinical NP experience
• Basic Life Support (BLS) certification
• Experience with EHR documentation (athenahealth preferred)
