Nurse Practitioner

📋 Description

• Provide ongoing primary and preventive care to older adult populations
• Manage and grow a dedicated patient panel within assigned communities
• Deliver relationship-centered care in community settings
• Collaborate with interdisciplinary clinical and operational teams
• Support preventive care, chronic disease management, and care coordination
• Utilize telehealth and in-person delivery to improve accessibility

🎯 Requirements

• Active, unrestricted Nurse Practitioner
• Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology)
• Active DEA license and CS registration as required
• 3+ years NP clinical experience, preferably with aging/complex populations
• Basic Life Support (BLS)
• Valid driver’s license with reliable transportation (car stipend provided)
