Nurse Practitioner (Remote, NJ License Required)

📋 Description

• Cover transitions of care for discharged patients via phone/video visits.
• Telephonic outreach and scheduled video/phone visits post-discharge.
• Medication reconciliation, symptom screening, and follow-ups.
• Build trust, explain Galileo’s care model, connect to services.
• Coordinate care across hospital/ED transitions, pharmacies, and providers.
• Collaborate with nursing team; support population health goals.

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics.
• Active NJ licensure; ability to obtain additional state licenses.
• Bonus licenses in TX, PA, IL, NY, MA, MN, LA, SC.
• 5+ years post-graduate experience in Primary Care or Geriatrics.
• Up to 2 years may be substituted by experience in Skilled-Nursing or LTC.
• Direct experience with Medicare/Medicaid populations with complex conditions.
• Motivational interviewing skills for behavior change.
• Comfortable in a digital-first environment with virtual care.
• Strong tech proficiency; Google Suite and Slack a plus.
• Excellent written and verbal communication skills.
• Bilingual English/Spanish; cultural fluency is a bonus.
