Nurse Practitioner (Remote, PA License Required)

📋 Description

• Cover care needs for the Transitions of Care program.
• Telephonic outreach and scheduled video/phone visits after discharge.
• Medication reconciliation, symptom screening, and follow-up labs/meds/supplies.
• Build trust, explain Galileo's care model, and connect patients to services.
• Coordinate care across hospital and ED transitions, pharmacies, and community providers.
• Collaborate with our nursing team and meet population health targets.

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics.
• Active PA licensure with ability to obtain additional state licenses.
• Bonus: licenses in TX, NJ, IL, NY, MA, MN, LA, SC.
• 5+ years post-graduate clinical experience in Primary Care or Geriatrics.
• Direct experience with Medicare and Medicaid populations managing complex conditions.
• Motivational interviewing skills.
• Comfort in a digital-first environment with virtual care delivery.
