Nurse Practitioner (Remote, SC License Required)

📋 Description

• Cover our care needs for Transitions of Care.
• Telephonic outreach and video/phone visits for recently discharged patients.
• Medication reconciliation, symptom screening, and follow-up labs/medications/supplies.
• Navigate conversations to build trust, explain Galileo's care model and connect patients to services.
• Coordinate care across hospital settings, ED transitions, and pharmacies.
• Collaborate with the nursing team.

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics
• Active SC licensure; ability to obtain additional state licensures
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Motivational interviewing skills
• Comfort with digital-first environment and virtual care
• Solid tech proficiency; Google Suite and Slack a plus
