Nurse Practitioner (Remote, SC License Required)

📋 Description

• Provide follow-up care to post-hospital discharge patients via phone/video.
• Cover Transitions of Care program needs.
• Telephonic outreach and scheduled video visits for recently discharged patients.
• Medication reconciliation, symptom screening, and follow-up labs/meds coordination.
• Build trust, explain Galileo's care model, and connect patients to services.
• Coordinate care across hospital, ED, pharmacies, and community providers.

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics.
• Active SC state licensure with ability to obtain additional licenses.
• 5+ years post-graduate clinical experience in Primary Care or Geriatrics.
• Direct experience engaging Medicare and Medicaid populations managing complex conditions.
• Motivational interviewing skills to help patients make behavioral changes.
• Comfortable in a digital-first environment with virtual care delivery; Google Suite/Slack a plus.
