Occupational Therapist

📋 Description

• Coordinate and provide restorative occupational therapy services.
• Collaborate with PACE physicians, rehab staff, and IDT to maximize independence.
• Assess participant needs and goals in facility and home.
• Develop and document care plans with the IDT.
• Deliver and document interventions per care plans.
• Provide active and passive exercise, gait and ADL training.

🎯 Requirements

• Bachelor’s degree in occupational therapy; master’s preferred.
• Two years in clinical setting with frail/elderly population preferred.
• Valid California OT License or license eligibility.
• CPR and First Aid certification within 180 days of hire.
