📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, coordination, communication, ADL and IADL.
• Assess cognitive, psychological, and motor performance; develop individualized plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations for better outcomes.
• Document observations, interventions, and evaluations using EMR tablet.

🎯 Requirements

• Current SC OT license.
• At least 1 year OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills; problem solving.
• Solid computer skills; EMR experience preferred.
