📋 Description

• Provide one-on-one OT services in senior living communities
• Conduct comprehensive evaluations and create treatment plans
• Utilize evidence-based interventions to improve safety and independence
• Collaborate with home health team for care transitions
• Educate patients and families on techniques and safety
• Document therapies in EMR and manage ADL assessments

🎯 Requirements

• Current New Jersey OT license
• Minimum 1 year OT experience
• Graduated from an AOTA-approved OT program
• Home health experience preferred
• Strong clinical, communication, and organizational skills
• Ability to work independently with a flexible schedule
