📋 Description

• Provide skilled occupational therapy services through home visits within designated territories.
• Perform comprehensive evaluations of patients' functional abilities, including strength, endurance
• Develop individualized treatment plans to maximize independence, safety, and functional outcomes.
• Assess home environments for safety concerns and recommend modifications or adaptive equipment to
• Identify and recommend assistive devices and environmental adaptations to improve patient function
• Collaborate with nursing and interdisciplinary team members to coordinate comprehensive patient

🎯 Requirements

• Current New Jersey Occupational Therapist (OT) license.
• Minimum of one year of recent Occupational Therapy experience.
• Prior home health experience preferred but not required.
• Graduate of an accredited Occupational Therapy program.
• Strong assessment, communication, organizational, and problem-solving skills.
• Ability to work independently and effectively manage time.
