Occupational Therapist, Full Time

📋 Description

• Occupational Therapist to join Habitat Health at our South LA PACE Center in a full-time capacity.
• Deliver evaluation, treatment planning, and direct therapy to support older adults in home and
• Collaborate with interdisciplinary teams to provide comprehensive care aligned with PACE model.
• Provide documentation and participate in program meetings to ensure quality care.
• Assist with ADLs, mobility, and home-based interventions as needed.

🎯 Requirements

• A minimum of 2+ years as a practicing OT.
• National Certification by the American Occupational Therapy Association or eligible for National
• Ability to work in healthcare, community, and/or home-based environments.
• Strong communication with participants, caregivers, and team members in person/phone/electronic
• Bilingual Spanish/English strongly preferred; other languages a plus.
