Occupational Therapist - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Revise the care plan with nursing staff as appropriate.

🎯 Requirements

• Current Pennsylvania Occupational Therapist license.
• Minimum one year OT experience; home care experience preferred but not required.
• Graduation from an accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
