Occupational Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of functional status, including muscle function, endurance, and ADL/IADL.
• Assess cognitive, psychological, and motor aspects; develop intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Collaborate with nursing to update the multidisciplinary care plan.

🎯 Requirements

• Current MD OT license.
• Minimum of one year of OT work experience.
• Graduate of AMA/AOTA-accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
