📋 Description

• Make home visits to clients in designated territories.
• Evaluate patients’ functional status: ADL/IADL, mobility.
• Assess cognitive, psychological, and motor performance; develop plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually assess and revise OT care plans with nursing.

🎯 Requirements

• Colorado OT license in good standing.
• 1+ year OT experience (home health preferred).
• Graduation from an OT program accredited by AMA/AOTA.
• Degree required: OT program graduation.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
