Make home visits to clients in designated territories.
Evaluate functional status: muscle function, endurance, vision, communication, ADLs/IADLs.
Assess cognitive, psychological, and motor aspects; develop individualized plans.
Evaluate home environment for hazards and barriers to independence.
Identify equipment needs and environmental adaptations.
Continually assess and revise the OT care plan; collaborate with nursing in the multidisciplinary team.

🎯 Requirements

Current North Carolina OT license.
Minimum one year OT experience.
OT program accredited by AMA and AOTA.
Prior home care experience preferred but not required.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem solving skills.
