  • Make home visits to clients in designated territories.
  • Evaluate functional status: muscle function, endurance, vision, communication, ADLs/IADLs.
  • Assess cognitive, psychological, and motor aspects; develop individualized plans.
  • Evaluate home environment for hazards and barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Continually assess and revise the OT care plan; collaborate with nursing in the multidisciplinary team.

🎯 Requirements

  • Current North Carolina OT license.
  • Minimum one year OT experience.
  • OT program accredited by AMA and AOTA.
  • Prior home care experience preferred but not required.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational, and problem solving skills.
