Occupational Therapist, (OT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environment for hazards for independence.
• Identify equipment needs and environmental adaptations.
• Revise care plan with nursing in the multidisciplinary team.

🎯 Requirements

• Current North Carolina Occupational Therapist license.
• One year of OT experience preferred; home care a plus.
• Accredited OT program completion as required.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
