Occupational Therapist, (OT) - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations of functional status incl. muscle func, endurance, coordination, communication
• Assess cognitive, psychological and motor performance; develop intervention plans.
• Evaluate home environment for hazards to independence.
• Identify equipment needs & environmental adaptations.
• Revise OT care plan and collaborate with nursing.

🎯 Requirements

• A current North Carolina Occupational Therapist license.
• Minimum one year OT experience; home care preferred.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
