📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of functional status (muscle function, endurance, coordination, communication
• Assess cognitive, psychological and motor performance; develop intervention plans.
• Evaluate home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Collaborate with nursing and multidisciplinary team; update care plans as needed.

🎯 Requirements

• Current OH Occupational Therapist license.
• Minimum 1 year OT experience.
• Graduation from an accredited OT program.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Commitment to home health care and patient education.
