Occupational Therapist- Visits

📋 Description

• Perform thorough evaluations of patients’ functional status and home safety for independent living.
• Assess cognitive, psychological, and motor aspects to develop individualized intervention plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations to enhance outcomes.
• Update and revise the OT care plan and collaborate with nursing in the multidisciplinary team.
• Educate patients and families on rehabilitative care to promote health and safety.

🎯 Requirements

• Licensed Occupational Therapist in Hawaii.
• Willingness to travel within assigned territory.
• Graduate from an AOTA/AMA-accredited OT program (transcript/diploma acceptable).
• 1 year of OT experience preferred.
