📋 Description

• Work in clinics to support CCM and RPM; Arabic knowledge preferred.
• Engage with patients in person to review care plans and goals.
• Serve as liaison between patients, clinicians, and care team.
• Assist clinic staff with enrollments, follow-ups, and monitoring.
• Prepare health data for visits to support care teams.
• Collaborate with care management to streamline delivery and improvements.

🎯 Requirements

• Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification required.
• 1+ years of clinical experience, preferably in care management.
• Strong communication and teamwork with clinical staff and patients.
• Proficiency in medical terminology and clinical processes.
• Highly organized, detail-oriented, able to multitask.
• Willingness to travel to Detroit-area clinics as needed.
