Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, ROM, transfer status, balance, and safety.
• Develop and implement individualized care plans with PT interventions.
• Continuously assess and adjust the PT care plan with the multidisciplinary team.
• Educate clients and families on rehabilitative care for safety and independence.
• Document observations, interventions, and evaluations in EMR with a touch pad tablet.

🎯 Requirements

• Current PA PT license.
• Minimum 1 year of PT work experience.
• Graduation from APTA- or CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
