Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, gait, balance.
• Develop and implement individualized care plans: manual therapy, gait training.
• Assess and revise care plan; collaborate on multidisciplinary care.
• Educate clients and families in rehabilitative care and independent living.
• Document observations, interventions, and evaluations in EMR on a tablet.

🎯 Requirements

• A current PA Physical Therapist license.
• A minimum of one year of PT experience.
• Graduate from an APTA/AMA-accredited program as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
