Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Assess mobility, ROM, strength, balance, and safety.
• Develop and implement individualized care plans incl. exercises and gait.
• Continuously assess and revise the PT care plan with nursing.
• Educate clients and families in rehab for independence.
• Document observations and interventions in EMR via touchpad.

🎯 Requirements

• Graduation from an APTA-approved PT program (transcript or diploma).
• Maryland Physical Therapist license.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
