Outpatient Physical Therapist- Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, gait, balance, strength.
• Develop and implement individualized care plans with manual therapy, gait, balance.
• Continually assess and revise PT care plans with the care team.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR on a tablet.

🎯 Requirements

• Maryland PT license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved PT program (or AMA CAAHEA).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
