Outreach Specialist

📋 Description

• Builds trusted relationships with healthcare providers and referral partners to drive referrals.
• Develops outreach strategies to raise awareness of Habitat Health's PACE program.
• Supports enrollment by guiding referrals through enrollment with the Outreach team.
• Delivers community engagement through presentations, events, and health fairs.
• Manages enrollment activities: reviews applications and communications.
• Identifies opportunities to expand referral sources using market insights.

🎯 Requirements

• Bachelor's degree in Business, Marketing, Healthcare Administration, Public Health, or equivalent.
• 1+ year in healthcare sales, business development, provider relations, outreach, or enrollment.
• Proven ability to build relationships with providers and partners to support enrollment.
• Strong organizational skills; manage multiple priorities and maintain accurate records.
• Strong communication and collaboration skills to educate providers and partners about PACE.
• Experience with Microsoft Office and ability to learn CRM and referral management systems.
