Outreach Specialist / Care Coordinator - Hampton Roads, Virginia

📋 Description

• Initiate outreach to new patients and connect to resources.
• Foster trust and improve patient engagement through empathetic communication.
• Review patient data from EHRs to optimize care and prevention.
• Communicate with primary care providers via multiple channels (EHR, in-person).
• Be the primary day-to-day contact and build collaborative provider relationships.
• This is a hybrid role requiring some on-site work in facilities.

🎯 Requirements

• Excellent verbal and written communication skills.
• Empathetic, credible communication with physicians and patients.
• Highly organized, self-motivated; customer service or sales experience.
• Experience with Electronic Health Records (EHRs).
• Comfort with Google Workspace and other tech platforms.
• Will travel up to 30% and attend a 3-week virtual training.
