📋 Description

• Lead day-to-day activities of care coordinators in the market.
• Oversee outreach, patient care coordination, and provider communication.
• Coach and mentor CCs to ensure workflow competency and compliance.
• Collaborate with cross-functional teams to enable seamless workflows.
• Review dashboards to track CC performance and optimize metrics.
• Hire, train, deploy, and support local CCs with Ops and Learning & Development.

🎯 Requirements

• Deep commitment to improving healthcare for underserved populations.
• Experience supervising teams of care coordinators or similar roles.
• Strong understanding of Medicaid/MCOs and access barriers.
• Detail-oriented with ability to monitor metrics and analyze data (Excel).
• Experience with healthcare data and Electronic Health Records (EHR).
• Willingness to travel within the Chicago area.
