📋 Description

Lead and supervise CCs in Sonoma, Orange, and San Bernardino counties.
Oversee daily CC tasks: outreach, care coordination, and provider communications.
Coach and develop CCs; ensure workflows and compliance.
Collaborate with local leads and care team to execute cross-team workflows.
Monitor CC performance via data dashboards and drive improvement.
Hire, train, deploy, and support CCs; mentor and grow talent.

🎯 Requirements

Supervise care coordinators or similar roles.
Cultural humility; knowledge of Medicaid/MCOs and access barriers.
Data-driven; proficient with Excel to analyze metrics.
Collaborative, cross-disciplinary teamwork.
Experience with healthcare data and/or EHR.
Residency in Sonoma/Orange/San Bernardino counties; travel up to 30%.
