📋 Description

• Provide care to patients, customizing treatment plan to individual patient needs
• Support and closely coordinate behavioral health care with the patient’s primary care provider and
• Screen patients for common behavioral health and substance use disorders using PHQ9, GAD7, Columbia
• Monitor patients for changes in clinical symptoms and treatment side effects/complications
• Support psychotropic medication management prescribed by PCPs
• Track patient follow-up and clinical outcomes using a registry, record encounters, and use the

🎯 Requirements

• Master’s Degree in Social Work, Counseling, or related field from accredited program
• Licensed to practice in Pennsylvania as independent clinician (LCSW/LICSW, LMHC, LMFT, LPC, or
• Minimum 3-5 years experience as licensed professional
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based
• Proficient in medical interpretation and documentation in English and Spanish (preferred)
• Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational
