PACE Pre-Enrollment Master of Social Work

📋 Description

• Conduct psychosocial assessments for prospective PACE participants daily.
• Approximately 95% of assessments conducted virtually via telehealth.
• Travel to applicants' homes or PACE centers for in-person evaluations as needed.
• Ensure applicants meet clinical criteria and Health and Safety requirements for community-based care.
• Collaborate with the interdisciplinary enrollment team to enable safe transitions.
• Advocate for older adults' autonomy and dignity throughout enrollment.

🎯 Requirements

• MSW degree required.
• 1+ year post-graduate clinical experience; 2+ years with older adults/geriatric populations.
• Strong knowledge of geriatric mental health (dementia, depression, anxiety).
• Psychosocial/biopsychosocial assessments and capacity/informed consent.
• Telehealth delivery experience with virtual assessments; in-person when needed.
• K​nowledge of PACE enrollment criteria and health/safety regulations.
