Part Time Registered Nurse (RN) - Home Care RAI Visits

📋 Description

• Work one-on-one with patients in their home to support independence.
• Conduct annual RAI and GHE assessments for Medicaid home-care clients.
• Evaluate functional abilities, health status, and care needs.
• Document findings and maintain state compliance with requirements.
• Collaborate with clients, caregivers, and interdisciplinary teams.
• Support the Medicaid reauthorization process for in-home services.

🎯 Requirements

• Current MO RN or LPN license in good standing.
• Current CPR certification.
• Strong organizational and communication skills.
