📋 Description

  • Act as liaison; maintain open communication with IDT, care team, and providers.
  • Assist with participant experience and service-recovery projects (e.g., surveys).
  • Facilitate communication among providers, families, participants, and caregivers.
  • Provide customer service calls to ensure satisfaction with IDT and appointments.
  • Support grievance process with high-touch service and IDT collaboration.
  • Attend required meetings and committees as directed.
  • Continue check-ins to prevent disenrollment and address concerns.

🎯 Requirements

  • High school diploma or equivalent; professional experience may substitute.
  • Bachelor’s degree in communications or relevant field preferred.
  • Excellent organizational, customer service, and communication skills with seniors and IDT.
  • Ability to present information to various groups and leadership.
  • Self-directed, adaptable, able to execute tasks in changing environments.
  • CPR and First Aid certification within 180 days; certification preferred at hire.
