Patient Advocate

📋 Description

• Build trusted, lasting relationships with older adults living in assigned communities
• Serve as the primary community representative for Matter Health
• Educate prospective patients about Matter Health's care model and available services
• Support patient outreach, enrollment, scheduling, and ongoing engagement
• Conduct in-person outreach, phone calls, and community events to connect with patients
• Partner closely with Advanced Practice Providers (APPs), Social Workers, Care Navigators, and

🎯 Requirements

• 1–2 years of experience in community outreach, patient advocacy, healthcare, social services
• Passion for serving older adults and underserved communities
• Outstanding communication and relationship-building skills
• Comfort working independently in a field-based environment
• Strong organizational and time-management skills
• Ability to collaborate effectively with clinical and operational teams
