Patient Advocate

📋 Description

• The Patient Advocate & Liaison (PAL) is the face of Matter Health in assigned communities.
• Field-based role building trusted relationships with older adults in affordable housing.
• Educate patients about Matter Health and assist with enrollment and follow-up.
• Coordinate care and help patients stay connected to needed services.
• Collaborate with NP, APPs, social workers, care navigators, and operations teams.
• Document activities and support patient retention and access to care.

🎯 Requirements

• 1–2 years in community outreach, patient advocacy, healthcare, or related roles.
• Strong communication and relationship-building skills.
• Independent, field-based work style with good organization.
• Valid driver’s license and reliable transportation.
• Experience with Salesforce, Athena, or healthcare outreach a plus; bilingual Spanish a plus.
