Patient Advocate

📋 Description

• Build trusted relationships with older adults in assigned communities
• Serve as the primary community representative for Matter Health
• Educate patients about Matter Health's care model and services
• Support outreach, enrollment, scheduling, and ongoing engagement
• Conduct in-person outreach, calls, and events to connect with patients
• Collaborate with APPs, Social Workers, Care Navigators, and Operations

🎯 Requirements

• 1–2 years in community outreach, patient advocacy, healthcare, social services, customer service
• Passion for serving older adults and underserved communities
• Excellent communication and relationship-building skills
• Comfortable working independently in a field-based setting
• Strong organization and time management
• Ability to collaborate with clinical and operational teams
