Peer Support Specialist

📋 Description

• Remote engagement with patients to navigate health and social needs
• Bridge between patients, providers, and community resources
• Support whole-person health and care coordination across systems
• Assist with referrals, scheduling, and follow-up care
• Document interactions and care activities in EHR systems
• Contribute to quality measures and value-based care goals

🎯 Requirements

• At least 3 years in patient navigation, care coordination, or peer support
• Experience with EHR systems and remote work
• Knowledge of medical terms, primary care workflows, social drivers
• Strong communication, empathy, and problem-solving skills
• Motivational interviewing or care navigation experience preferred
• Ability to manage multiple tasks independently
