Per Diem Care Manager, RN (Downstate, NY / Field-Based)

📋 Description

• Conduct initial client assessments to determine care needs and develop care plans.
• Train and supervise caregivers in personal care, companionship, and daily living activities.
• Monitor client well-being and report changes to healthcare professionals.
• Educate clients and families on care strategies, safety, and daily living skills.
• Ensure compliance with agency policies and state regulations in non-skilled care.
• Maintain accurate client care documentation and confidentiality.

🎯 Requirements

• BSN with NYS license or compact license; or ADN with homecare experience.
• 1-2 years nursing experience in homecare.
• Geriatric or disability care experience.
• Willing to travel within the five boroughs; valid driver's license.
• Knowledge of state and federal home health regulations.
• Strong interpersonal, verbal/written communication; organizational skills; EHR proficiency.
• Experience with Uniform Assessment System (UAS).
• Bilingual in Spanish, Russian, Chinese, Korean, or Bengali preferred.
