Per Diem Clinic RN

📋 Description

• Deliver direct participant care in clinic and during home visits.
• Support clinic operations and clinical excellence protocols.
• Assess participant needs and support treatment plans.
• Provide education and ensure accurate documentation.
• Participate in care team huddles and care coordination.
• Ensure timely vitals, triage, and nursing procedures as assigned.

🎯 Requirements

• Current California RN license.
• BLS certification.
• Valid driver's license and reliable vehicle.
• IV and blood draw certification.
• Minimum 2 years of complex clinical nursing experience (ambulatory/home health/PACE preferred).
• Proficiency with EHR systems and mobile documentation.
