Per Diem On-Call Registered Nurse (RN)

📋 Description

• Respond to after-hours patient needs within your assigned coverage area
• Deliver skilled nursing care, including medication administration, wound care, treatments, and
• Monitor patient conditions and communicate changes to the Clinical or Client Services Manager
• Follow physician orders and ensure timely, accurate documentation
• Coordinate care with physicians, caregivers, and the interdisciplinary team
• Document all patient care using an electronic medical record (EMR) system

🎯 Requirements

• Current Registered Nurse (RN) license in the State of Delaware
• Minimum of one year of recent, verifiable Home Health nursing experience
• Experience completing OASIS documentation and Start of Care visits required
• Strong assessment, clinical decision-making, and communication skills
• Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus
• Reliable transportation and ability to travel throughout Somerset, Union, Middlesex, and Monmouth
