Pharmacy Care Coordinator

📋 Description

• Conduct high-volume outbound telephone outreach to patients
• Deliver clear, standardized messaging over the phone
• Obtain and document patient consent by phone
• Make necessary changes in the EHR based on patient decisions
• Manage outreach queues and follow-up cadences
• Accurately document all call activity and coordination activities

🎯 Requirements

• Current Medical Assistant certification (CMA/RMA)
• Experience in telephonic, call-center, or high-volume outbound patient/customer outreach environment
• Strong interpersonal and communication skills
• Experience in healthcare, pharmacy, care coordination, or other patient support roles
• Proficiency with electronic health record (EHR/EMR)
• Strong organizational skills and attention to detail
