Pharmacy Care Coordinator, MA (Contract to Perm)

📋 Description

• Serve as the onsite Evergreen representative at practice sites, meeting patients after the provider
• Engage patients in a clear, standardized way — using approved messaging and supporting materials to
• Obtain and document patient consent, and capture the information needed to enroll the patient
• Make the necessary changes in the EHR based on the patient's decisions — updating the patient's
• Conduct telephonic outreach to patients who do not have an office visit, or who cannot be supported
• Accurately document all patient interactions, outreach attempts, accepts/declines, and coordination

🎯 Requirements

• Current Medical Assistant certification (CMA/RMA).
• Patient-facing healthcare experience in a clinical, pharmacy, care coordination, or patient support
• Comfort and proficiency working within an electronic health record (EHR/EMR).
• Strong interpersonal and communication skills, with the ability to build trust and engage patients
• Ability to work onsite at practice locations, with reliable transportation as required.
• Working knowledge of HIPAA and a demonstrated commitment to protecting patient privacy and PHI.
