📋 Description

• Provide telephonic/video and SMS outreach to Medicaid patients in California.
• Collaborate with a multidisciplinary team to wrap around primary care.
• Support patients and providers around transitions of care.
• Deliver clinical services that support day-to-day operations and safety.
• Use analytics dashboards to identify at-risk patients and prevent events.
• Lead and mentor pharmacists and pharmacy technicians.

🎯 Requirements

• PharmD with CA license in good standing.
• Completion of accredited PGY1 residency training in related discipline.
• 5+ years of ambulatory care with vulnerable populations.
• Experience in multi-disciplinary care management.
• Positive referrals from prior colleagues.
